The Georgia Pediatric Program (GAPP) is a Medicaid-funded initiative that provides skilled nursing and personal care services to medically fragile children under age 21. These services are delivered in the comfort and safety of the child’s home at no cost to families. Medicaid-eligible families may apply, with approval determined by the child’s specific medical needs.

Daily Care & Mobility
- Personal Hygiene: Comprehensive assistance with bathing, dressing, and hygiene
- Mobility Support: Expert wheelchair assistance and routine turning or repositioning to ensure comfort
- Home Comfort: Regular changing of bed linens and support with activities of daily living (ADLs)
Clinical & Medical Support
- Medication Management: Precise administration of prescribed medications
- Enteral Nutrition: Gastrostomy tube (G-tube) feeding, cleaning, and site maintenance
- Vitals & Monitoring: Continuous observation and tracking of vital signs to monitor health status
Advanced Respiratory Care
- Tracheostomy Management: Professional trach care, cleaning, and suctioning
- Respiratory Therapy: Administration of breathing treatments and manual bagging via trach or mouth
Care Coordination
- Clinical Communication: Detailed gathering of treatment plan data and immediate reporting of health changes to doctors or nurses

Initial Consultation

Eligibility Review
We will help you verify Georgia Medicaid or TEFRA eligibility to ensure your services are covered.
In-Home Assessment
A Registered Nurse (RN) will visit your home to conduct a clinical evaluation and determine the required level of care.
Caregiver Matching
Once approved, we coordinate your customized treatment plan and match your family with a qualified nurse or aide.
Please note that the intake and evaluation process typically takes up to 30 days to complete.
